If you have a family doctor it is appropriate to obtain a referral for a sports medicine consultation prior to coming for your assessment. This will provide our doctor important medical information about you prior to your assessment. It will also provide your doctor with a record of our assessment. This will ensure your medical records with your doctor are complete. If your doctor is not one of our regular referring doctors, you can download a "patient referral form" from the link below and forward the form to your doctor to fill out and fax to us. Your doctor may require that you visit them if they have never assessed you for the condition for which you are seeking advice. If you experience any difficulties organizing a consultation, please call Christie Langevin our secretary, she will be happy to help you out.  |
